  No primary care physicians required; self-referral to specialist allowed
  • PPO 200 - $200 deductible; copays; 90% in network coinsurance
  • PPO 500 - $500 deductible; copays; 80% in network coinsurance
  • PPO 1000 - $1000 deductible; copays; 70% in network coinsurance
  • High Deductible Health Plan with Health Savings Account (HSA)
    $2000 deductible; 100% coinsurance after deductible other than prescription copays; company contribution to HSA

Vision Insurance

• Vision Insurance offered by Vision Service Plan® (VSP) (FT & PT 20+ hours)
  • In and out of network coverage
  • Copays for exam and glasses or contact lenses with in network provider
  • Partial reimbursement for exam and glasses or contact lenses with out of network provider
